Our rehabilitation department delivers the post-acute continuum of inpatient, outpatient and long-term care services. The department consists of a team of highly skilled occupational therapists, physical therapists and speech-language pathologists who provides short-term rehab in our skilled nursing care center. The team works closely with nursing, other interdisciplinary team members as well as residents and their families.

• Specialized equipment treatment

Our team provides services to our long-term care residents as needs arise, working directly with the nursing staff to create a program specific to individual resident’s needs — helping each regain an optimal level of function.

Our rehab department also provides services to our residents in our assisted living community, addressing illnesses, injuries and specific needs such as pain management, fall prevention and management, balance management and training, home safety, and mobility issues.

The department is also capable of providing outpatient services for those in the community.
