Description
St. Leonard has an new opportunity for a part-time Licensed Social Worker to join our team.
Our beautiful 240 acre campus is one of the largest Continuing Care Retirement Communities in the United States. St. Leonard features independent living, assisted living, dementia care, skilled nursing, rehabilitation services, and adult day care as well as a community wellness center that encompasses a fitness center, and restaurant.
Working with the interdisciplinary team, the Social Worker assesses and promotes the social and emotional health and well-being of residents and their families. The Social Worker provides service to adults and older adults with the goal of affirming the resident’s integrity, growth, and control over the qualities of their life.
Essential Job Specific Duties/Responsibilities:
- Demonstrates knowledge of skills necessary to identify specific resident needs and
provide appropriate services. - Participates in development of interdisciplinary plan of care.
- Actively develops one-on-one relationships with residents and families as needed to be
able to meet individual needs. - Provides education and resources, as well as support to residents, families, and staff.
- Participates and takes a leadership role as needed in various activities.
Job Requirements
- Bachelor’s Degree in Social Work from an accredited college or university preferred
- Current license for Licensed Social Worker for the State of Ohio
- Currently certified in CPR
- Previous experience with mature adults preferred
