Job Responsibilities:
The Nurse Liaison is an RN who advocates and promotes the facility’s services to hospitals, case workers, discharge planners, physician offices, and other clinical providers. The Nurse Liaison follows up on hospital referrals, evaluates residents for proper placement, explains the facility’s services, and maintains and develops referral sources.
Job Requirements
Valid Wisconsin RN license
Two years’ experience in assisted living, senior housing, or senior healthcare
